       Centennial High School: 330 E Laurel, Fort Collins, CO 80524

• Affordable: 

1. $50 cash fee for sports physicals
2. Sliding fee scale available for qualifying families 
3. We accept Medicaid, CHP+, and most private insurance, with no co-pay or deductibles required
4. No student is ever turned away due to an inability to pay for services at the Health & Wellness Centers

The Health & Wellness Centers are operated by Every Child Pediatrics, the state’s largest non-profit pediatric provider serving over 24,000 youth annually in 12 Colorado locations. Our School-Based Health Clinics offer high quality, youth-centered care and are part of a nationally recognized, best practice model designed to remove financial, transportation and other barriers. We reduce lost classroom time and keep kids healthy and ready to learn!
